October Flu Shots
WellQuest is once again offering a discounted flu shot clinic during the monthly meeting to all members. The date is set for October 24th from 11:30 - 12:00 and again at 1:00 - 1:20. We are distributing a flu shot consent form ( downloadable pdf below ) which each participating member will need to complete and return during the appropriate provided times. Be prepared to pay by check made payable to WellQuest for the anount of ( twenty-five dollars ) $25.00. Please wear clothing that will allow your arm to be easily accessible and remember, by choosing to get a flu shot with WIN you’re saving ( fourteen percent ) 14% off the original price.
For those planning on receiving a flu shot we ask that you RSVP Ashley Spurlock ( ashley.spurlock at wellquest.md ) on or before October 22nd to ensure availability.
